You have been shortlisted for the board-conferred Honorary Fellow (PCI) recognition. To continue, please provide the following. This step exists only to verify your identity; it does not involve any fee and does not guarantee recognition.

How your information is handled

Purpose & minimisation. Your photograph and ID are used solely to verify your identity for honorary recognition. We do not record any ID number or document detail — only the image is retained, and only for as long as needed for this purpose.
Secure storage & access. Documents are stored securely and are accessible only to the Institute’s board/reviewers on a strict need-to-know basis, and handled confidentially.
Retention & deletion. Your documents are deleted after the verification decision (or sooner on request), subject to any legal retention obligation.
International processing. The Institute operates globally; your information may be processed or stored in other countries with appropriate safeguards. If you are in Saudi Arabia, the United States, Pakistan or elsewhere, this is done in line with applicable data-protection law and our Privacy Policy and Data Protection Policy.
Your choice. Providing these is voluntary; you may decline or withdraw at any time, in which case your honorary application may not proceed.

I declare that, to the best of my knowledge, I have not been involved in, convicted of, or currently facing undisclosed legal proceedings for, serious criminal activity that is materially relevant to PCI’s honorary recognition. I understand PCI does not require details of any such matter here, that I may raise anything I wish the board to consider by contacting the Institute directly, and that a misleading declaration may lead to refusal or withdrawal of recognition. *
